Job summary

Elevance Health seeks a Field Advanced Practice Provider to deliver high-quality, home-based care across a designated Ohio territory, including Xenia, Cedarville, and Yellow Springs. You will collaborate with physicians to develop complex care plans and support patients and families in alignment with goals of care.

The role requires an MS in Nursing, an active NP/PA license, and DEA and CPR credentials. Travel to client sites is expected as part of routine duties.

Qualifications

• Requires MS in Nursing.
• Must hold an unrestricted NP license or PA license in applicable state(s).
• DEA registration or eligibility required.
• CPR certification required.

Responsibilities

• Provides services in home settings to deliver care.
• Develops and implements care plans for adults with chronic/complex conditions.
• Gathers history, performs exams, and develops treatment plans.
• Coordinates with patients, families, and inter-disciplinary team.
• Educates patients/families about medications, adherence, and prognosis.
• Maintains medical records per state regulations and policy.
