Qualifications

MS in Nursing is required to practice as an NP.
Active, national NP certification is required.
Ohio Family or Adult NP license is required.
RN license must be active and unrestricted.
2+ years of experience managing complex care.
Experience with EMR systems.

Responsibilities

Provide primary and urgent care via telephone and telehealth for home- and community-based patients.
Develop and implement plans of care for adults with chronic/complex conditions.
Gather history and exams, order diagnostics as needed, and implement treatment plans.
Close gaps in care and coordinate with inter-disciplinary team.
Educate patients on medications, adherence, and prevention.
Maintain medical records in compliance with state laws and policies.
Prescribe medications as allowed by state authority.
Participate in continuing education as required by state/certifying body.
