Job summary

Adaptive Home Health in Texas is seeking a Field-based Occupational Therapist to deliver skilled OT visits and in-home assessments. You will build Plans of Care focused on ADLs/IADLs, safety, and independence, and use adaptive equipment and home modifications as needed.

You will collaborate with nursing and the broader care team, document promptly in Homecare Homebase, and travel across assigned territory. A Texas OT license is required and you will work full-time with occasional weekend

Qualifications

• Active Texas OT license in good standing.
• Strong clinical judgment and field independence.
• Clear documentation and communication.
• Current driver's license and daily travel.
• Ability to transport basic therapy equipment.

Responsibilities

• Complete in-home OT evaluations and visits.
• Develop and execute plan of care for ADLs/IADLs.
• Recommend adaptive equipment and home modifications.
• Coordinate with nursing and care team.
• Document in HCHB promptly and thoroughly.
