Job summary

Adaptive Home Health is seeking a licensed Occupational Therapist (OT) to deliver in-home evaluations and skilled OT visits across Texas. You will build clear plans of care focused on ADLs/IADLs, safety, and functional independence, while coordinating with nursing and the care team.

This field-based role offers schedule flexibility, autonomy, and the opportunity to impact access to high-quality home health care. Texas license and daily travel across territory are required.

Qualifications

• Active Texas OT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Comfort escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Competency in core home health OT scope (ADL/IADL training, home safety).

Responsibilities

• Complete in-home OT evaluations and deliver skilled OT visits.
• Build and execute a care plan focused on ADLs/IADLs, safety, and independence.
• Recommend and train on adaptive equipment and home modifications.
• Coordinate with nursing and the broader care team when needs change.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
