Job summary

Adaptive Home Health is seeking an Occupational Therapist to join our field-based team across Texas. You will perform in-home evaluations, deliver skilled OT visits, and develop a plan of care centered on ADLs/IADLs, safety, and functional independence.

You will recommend adaptive equipment, coordinate with nursing, and document thoroughly in Homecare Homebase. A Texas OT license and ability to travel daily within the assigned territory are required, with a flexible schedule and supportive

Qualifications

Active Texas OT license (in good standing).
Strong clinical judgment and ability to work independently in the field.
Comfort escalating changes in condition to RN/MD and documenting rationale clearly.
Strong communication and documentation.
Reliable follow-through on orders, frequencies, and patient goals.
Current driver’s license and comfort with daily travel across assigned territory.
Able to carry/transport basic therapy equipment as needed.
Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education).

Responsibilities

Complete in-home OT evaluations and visits.
Develop a plan of care focused on ADLs/IADLs, safety, and functional independence.
Recommend and train on adaptive equipment and home modifications.
Coordinate with nursing and the broader care team as conditions change.
Document promptly and thoroughly in Homecare Homebase (HCHB).
