Job summary

Adaptive Home Health is seeking an Occupational Therapist to deliver in-home OT evaluations and skilled visits across Texas. You will develop a plan of care focused on ADLs/IADLs, safety, and functional independence, and train patients on adaptive equipment and home modifications.

As a field-based clinician, you will coordinate with nursing and the broader care team, document clearly, and travel daily within your territory while maintaining autonomy and impact on patient outcomes.

Qualifications

• Active Texas OT license in good standing.
• Strong clinical judgment and ability to work independently.
• Excellent communication and documentation skills.
• Reliable daily travel across assigned territory.
• Ability to carry and transport basic therapy equipment.

Responsibilities

• Complete in-home OT evaluations and deliver skilled visits.
