Job summary

Adaptive Home Health, LLC is seeking an Occupational Therapist (OT) for field-based, full-time work across Texas. You will perform in-home OT evaluations, develop and execute plans of care focusing on ADLs/IADLs, safety, fall prevention, and functional independence, and recommend adaptive equipment or home modifications as needed.

Qualifications

• Active Texas OT license (in good standing).
• Strong clinical judgment; ability to work independently in the field.
• Clear communication and documentation.
• Reliable follow-through on orders, frequencies, and goals.
• Valid driver's license and daily territory travel.
• Ability to transport basic therapy equipment.

Responsibilities

• Complete in-home OT evaluations and deliver skilled OT visits.
• Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
• Recommend and train on adaptive equipment and home modifications as needed.
• Coordinate with nursing and the broader care team when condition or needs change.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
