Job summary

Adaptive Home Health in Texas is seeking a licensed OT to deliver in-home evaluations and skilled visits across the field-based territory. You will implement ADL/IADL-focused plans, enhance safety, and coordinate with nursing and the broader care team for changes in patient needs.

You’ll work independently with strong communication, documentation, and reliability, carrying basic therapy equipment, traveling daily, and adhering to the Texas licensure standards.

Qualifications

• Active Texas OT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication and thorough documentation.
• Reliable follow-through on orders, frequencies, and patient goals.

Responsibilities

• Complete in-home OT evaluations and deliver skilled OT visits.
• Build and execute a plan of care focused on ADLs/IADLs, safety and independence.
• Recommend and train on adaptive equipment and home modifications as needed.
• Coordinate with nursing and the broader care team when conditions change.
• Document promptly and thoroughly.
