A senior healthcare provider is seeking a Nurse Practitioner to deliver field-based primary care in patient homes. This position involves managing chronic diseases, collaborating with interdisciplinary teams, and preventing emergency visits. Ideal candidates will have an advanced degree, active NP license in Ohio, and reliable mobility for patient visits. Qualifications

• Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.
• Active, unencumbered NP license and Board Certification (ANCC or AANP).
• Valid driver’s license and vehicle for local field-based travel.

Responsibilities

• Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home.
• Create longitudinal care plans that move the needle for high-risk patients.
• Partner with an interdisciplinary team to ensure seamless, safe care transitions.
• Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.
