Qualifications

• Advanced degree: Master’s Degree (NP) with a focus on Adult, Geriatric, Acute Care or Family.
• Active NP license and Board Certification (ANCC or AANP) required.
• Valid driver’s license and vehicle for local field travel.

Responsibilities

• Deliver field-based primary care visits and exams in patients’ homes.
• Create longitudinal care plans and manage chronic diseases.
• Collaborate with interdisciplinary teams for safe care transitions.
• Prevent ER visits by treating acute needs at home.
