Job summary

WellBe Senior Medical is seeking a Nurse Practitioner for home health in Mt. Vernon, Ohio. This part-time role focuses on adult medicine with field-based primary care visits and comprehensive exams.

The NP will work with an interdisciplinary team to manage chronic diseases, optimize care transitions, and prevent ER visits, all in the patient’s home setting. Must hold an NP license with board certification and have reliable local travel ability.

Qualifications

• Master’s Degree (NP) with a focus on Acute Care, Adult, Geriatric, and/or Family.
• Active, unencumbered NP license and board certification (ANCC or AANP).
• Reliable mobility with valid driver’s license and vehicle for local field-based travel.

Responsibilities

• Deliver Field-Based Care: conduct comprehensive primary care visits and exams in patients’ homes.
• Manage Chronic Disease: create longitudinal care plans for high-risk patients.
• Collaborate Across the Continuum: partner with an interdisciplinary team for safe care transitions.
• Prevent ER Visits: treat acute needs at home to keep seniors healthy and out of the hospital.

Education
