St. Luke's Health Network, Inc. is seeking a Full-time Registered Nurse for the ACT program based in Pottstown, PA.

The role focuses on delivering in vivo services to ACT participants in homes and community settings, including medication delivery and crisis on-call support. The RN will collaborate with psychiatrists, nurses, and team leaders to develop treatment goals, provide counseling, conduct health assessments, and ensure documentation accuracy in the MAR.

Qualifications

• RN licensure in Pennsylvania and current practice status.
• Experience delivering home-based ACT services preferred.
• Ability to coordinate care across interdisciplinary team.

Responsabilités

• Meet ACT participants in homes and community venues to provide in vivo services.
• Deliver medications and observe ingestion per team directives.
• Provide treatment goal development, counseling, and crisis on-call coverage.
• Document services in the electronic health record per standards.
• Work weekend, evening, and holiday shifts as needed.
• Coordinate with medical providers to ensure timely health care.
