Job summary

Elevance Health is seeking an LTSS Service Coordinator - RN Clinician to manage member care plans and coordinate services across multiple counties in Indiana. The role requires an active RN license and experience in chronic illness management, with travel to client sites as needed.

The position emphasizes collaboration with physicians, social workers, and other licensed staff to optimize health outcomes and ensure appropriate utilization of benefits.

Qualifications

• HS diploma or equivalent.
• Minimum of 3 years experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
• Current, active valid and unrestricted RN license in applicable state(s) required.
• May require state-specified certification based on state law and/or contract.

Responsibilities

• Manages member cases within licensure scope and develops the care plan in collaboration with the member and care team.
• Performs telephonic or face-to-face clinical assessments for identification, evaluation, coordination and management of member needs.
• Provides direction to non-RN clinicians (LPN/LVN, LSW, LMSW, etc.) and assigns appropriate tasks to coordinate services.
• Interfaces with Medical Directors, Physician Advisors and interdisciplinary teams on care management treatment plans.
• Assists in problem solving with providers, claims or service issues and supports authorization/referrals within benefits structure.
