Qualifications

• Master's degree in Mental Health or Social Work or Nursing is required.
• 2–4 years of related behavioral health case management experience.
• Preferred 3+ years direct clinical behavioral health experience in case management or care coordination.

Responsibilities

• Develops and coordinates care plans for high-risk members with mental/behavioral health needs.
• conducts home visits to assess needs and coordinate resources.
• Educates members and families on services, benefits and treatment options.
• Documents activities in the care management system and ensures compliance with guidelines.
