Job summary

Qualifications

• Master's degree in Mental Health or Social Work or nursing degree with BH experience.
• 2–4 years of related behavioral health experience.
• Preferred 5+ years in Behavioral Health Case Management.
• Licensed Behavioral Health Professional or RN per state requirements.
• Experience with Medicaid managed care context is a plus.

Responsibilities

• Develops and coordinates holistic care management for populations with behavioral health needs.
• Evaluates member service needs and develops care/service plans.
• Provides home-based visits up to 75% monthly travel.
• Educates members and families on services, benefits, and treatment options.
• Coordinates referrals and access to community resources.
• Maintains documentation in electronic care management system.
• Ensures compliance with clinical guidelines and state/federal rules.
