Job summary

Centene is seeking a Behavioral Health Case Manager to develop, assess, and coordinate care for members with significant mental/behavioral health needs in the Corsicana area. The role involves evaluating high-risk members, educating families, and coordinating services and resources across medical, behavioral, and social domains.

Qualifications include a Master’s in Mental Health or Social Work (or RN with behavioral health experience) and 2–4 years of related case management experience.

Qualifications

• Master's degree in Mental Health or Social Work or RN with behavioral health experience.
• 2-4 years of related behavioral health case management and care coordination experience.
• Licensed Behavioral Health Professional or RN based on state requirements.

Responsibilities

• Evaluates the needs of the most complex and high-risk members with mental/behavioral health needs and recommends a plan of care.
• Acts as liaison between member/family, physician, and facilities/agencies.
• Supports members with mental/behavioral health needs, including serious mental illness.
• Performs frequent home/site visits to assess needs and coordinate resources.
• Educates members and caregivers on care plans, benefits, treatments, and services.
