Job summary

Centene Management Company LLC is seeking a field-based Care Manager in Kansas to assess member needs and coordinate long-term care plans with families and care providers. You will monitor outcomes and connect members with community resources to ensure access to services.

The role requires a Bachelor's degree and 2–4 years of related experience, with strong communication, time management, and team collaboration skills. Monday–Friday schedule with varying hours based on program needs.

Qualifications

• Requires a Bachelor’s degree and 2–4 years of related experience.
• Experience with HCBS waivers and behavioral health preferred.
• Ability to work independently in a community-based environment.

Responsibilities

• Evaluates the needs of the member, the resources available, and recommends and/or facilitates the plan for the best outcome.
• Assists with developing ongoing long-term care plans/service plans and identifies providers and community resources.
• Coordinates between member/family/caregivers and care provider team to ensure services are accessible.
• Provides resource support to members and families/caregivers for various needs based on assessments and plans.
• Monitors care plans and outcomes, and provides recommendations based on member needs.
• Interacts with long-term care providers and partners to ensure member needs are met.
• Collects and maintains long-term care member information to ensure compliance with regulations.
• May perform home or site visits to assess member needs and collaborate with providers.
• Provides and/or facilitates education to long-term care members on procedures and options.
• Provides leadership feedback to improve quality of care and service delivery.
• Performs other duties as assigned.
