Job summary

Concierge Home Care is seeking an experienced Registered Nurse (RN) Case Manager to provide in-home clinical oversight in Rochelle Park, NJ. This field-based role requires an organized, independent RN who builds strong client relationships while delivering individualized care in clients' homes.

Responsibilities include conducting in-home assessments, creating care plans, obtaining physician orders, monitoring conditions, and supervising Home Health Aides.

Qualifications

• Active New Jersey RN license.
• Minimum of 2 years of RN experience in acute care, home care, or case management.
• Experience with ventilator and tracheostomy care, G-tube/PEG tube management, IV infusions, and wound care.
• Current BLS Certification.
• Recent physical examination within the past year.
• Recent PPD or QuantiFERON test, or chest X-ray within past five years, if applicable.
• MMR immunization documentation.
• Valid driver's license and reliable transportation.
• Authorized to work in the United States.

Responsibilities

• Conduct comprehensive in-home nursing assessments.
• Develop, implement, and oversee individualized plans of care.
• Obtain and maintain physician orders in compliance with New Jersey regulations.
• Monitor client conditions and promptly communicate significant changes.
• Complete accurate and timely clinical documentation.
• Perform Home Health Aide supervisory visits and ensure quality of care.
• Provide clinical guidance and support to Home Health Aides and Nurses.
• Promote client safety, independence, comfort, and overall well-being.
