Job summary

Concierge Home Care is seeking an experienced Registered Nurse (RN) Case Manager to provide in-home clinical oversight in Monroe, NJ. This field-based role suits an organized, independent RN who enjoys building client relationships while delivering personalized, high-quality care in clients’ homes.

You will conduct home nursing assessments, develop care plans, obtain physician orders, monitor conditions, and supervise Home Health Aides to ensure safety and quality of care for diverse patient

Qualifications

• Active New Jersey RN license.
• Minimum 2 years RN experience in acute care, home care, or case management.
• Experience with ventilator/tracheostomy care, G-tube/PEG tube management, IV infusions, and wound care.
• Current BLS certification.
• Current physical examination (within the past year).
• PPD or QuantiFERON test within past year, or chest X-ray in past five years, if applicable.
• MMR immunization documentation.
• Valid driver's license and reliable transportation.
• Authorized to work in the United States.

Responsibilities

• Conduct comprehensive in-home nursing assessments.
• Develop, implement, and oversee individualized plans of care.
• Obtain and maintain physician orders in compliance with New Jersey regulations.
• Monitor client conditions and promptly communicate significant changes.
• Complete accurate and timely clinical documentation.
• Perform Home Health Aide supervisory visits and ensure quality of care.
• Provide clinical guidance and support to Home Health Aides and Nurses.
• Promote client safety, independence, comfort, and overall well-being.
