Job summary

VitalCaring is seeking a Hospice RN to lead patient-centered care in field-based settings, guiding the plan of care, symptom management, and comfort-focused nursing aligned with patient goals.

You’ll work autonomously with an interdisciplinary team, educate and support families, coordinate care, and ensure compassionate, dignity-driven end-of-life services in Garfield, Major, Alfalfa, and Kingfisher counties.

Qualifications

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care
• Ability to manage caseload independently
• Excellent communication in sensitive situations
• Basic EMR proficiency
• Valid driver's license and reliable transportation

Responsibilities

• Lead the plan of care and manage symptoms with a focus on comfort
• Develop and manage individualized, comfort-focused care plans
• Deliver skilled nursing care with pain and symptom control
• Act as case manager coordinating care across the IDT
• Collaborate with physicians, social workers, chaplains, and aides
• Monitor patient condition and adjust care plans for comfort
• Provide education and emotional support to patients and families
• Build trusting relationships in deeply personal moments
• Maintain accurate, timely documentation and participate in care planning
