Responsibilities

• Completes initial patient assessment and history to determine home care needs.
• Regularly re-evaluates patient nursing needs and updates the plan of care.
• Initiates and revises the care plan based on patient status and goals.
• Coordinates patient care as Case Manager with the Clinical Supervisor or designee.
• Documents per procedures and communicates with primary physician and family.
