Qualifications

• Prior experience in hospice, home health, healthcare sales, case management, or community liaison role preferred.
• Exceptional relational drive and genuine connection to the work.

Responsibilities

• Build and manage a referral territory with healthcare partners.
• Educate referral sources about clinical capabilities and patient criteria.
• Meet with families to explain hospice benefits.
• Drive admissions growth through qualified referrals.
• Maintain accurate activity tracking in CRM.
• Collaborate closely with clinical team for seamless hand-offs.
• Represent Ovation in the community with care.
