Zusammenfassung

Caringhand Home Health Solutions, Inc. is seeking a compassionate Registered Nurse (RN) to provide skilled nursing care to patients in their homes across California. You will assess patient needs, develop care plans, administer medications, educate families, and coordinate with physicians to ensure safe, independent living at home.

The role is field-based, per diem, with travel to patients’ homes. This is not telehealth; remote designation applies to scheduling rather than remote work.

Qualifikationen

• Graduate of an accredited school of nursing.
• Current, active RN license in California.
• At least 1–2 years of recent acute care experience in an institutional setting.
• Current CPR certification.
• Valid driver’s license and access to a reliable vehicle insured as required.
• Excellent observation and clinical assessment skills.
• Strong verbal and written communication skills.
• Effective problem‑solving and basic mathematical skills.
• Ability to perform nursing duties per competency requirements.
• Ability to work independently and coordinate patient care responsibilities.
• Ability to perform physical duties of patient care with accommodations.

Aufgaben

• Conduct initial patient and family assessments to determine home health care needs.
• Regularly reassess patients’ nursing needs and evaluate condition changes.
• Initiate and revise individualized plans of care as needs change.
• Establish nursing diagnoses and develop care plans.
• Develop patient-centered goals and interventions.
• Involve patients and families in care planning and decision-making.
• Administer medications and treatments as prescribed.
• Implement preventive and rehabilitative nursing procedures.
• Counsel patients and families on nursing needs.
• Educate patients and caregivers based on assessments and plan of care.
• Identify discharge planning needs and arrange prior to discharge.
• Serve as case manager for assigned patient caseload.
• Prepare accurate clinical notes and update primary physician every 60 days.
• Communicate changes to physicians and obtain orders as required.
• Coordinate with community healthcare professionals.
• Participate in on-call duties and ensure equipment/supplies availability.
• Instruct and supervise home health aides every two weeks.
• Perform other duties to support patient care.
