Top Echelon Software, LLC is seeking an experienced Home Health RN Case Manager for a field-based role serving the Dallas area and Fort Worth region. This position requires an active Texas RN license, recent home health case management experience, strong assessment skills, and the ability to manage caseloads in the field.

You will perform skilled nursing visits, develop plans of care with physicians and families, complete OASIS and visit notes, and maintain compliance with home health

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.

Responsabilités

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
