Job summary

Spoon River Home Health Services is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate care for patients in their homes. This full-time, field-based role involves independent field work with collaboration from physicians and the interdisciplinary team to ensure safe, effective home health care.

Responsibilities include assessments, care planning, skilled visits, patient education, and comprehensive documentation.

Qualifications

• Current RN license (Illinois)
• Minimum of 1 year nursing experience
• Strong time management and organizational skills
• Dependable and self-motivated
• Committed to delivering compassionate, person-centered care
• Home health experience preferred, but not required

Responsibilities

• Travel to patients' homes to provide skilled nursing care
• Complete patient assessments and ongoing evaluations
• Develop, implement, and update individualized plans of care
• Perform skilled nursing visits based on patient needs and physician orders
• Monitor changes in patient condition and communicate with physicians and the care team
• Educate patients and families about medications, diagnoses, treatments, and care needs
• Coordinate services with therapists, caregivers, physicians, and other healthcare professionals
• Complete timely and accurate clinical documentation
• Help ensure continuity, quality, and safety of care throughout the patient's home health experience
• Follow agency policies, clinical standards, and regulatory requirements
