Caringhand Home Health Solutions, Inc. is seeking a compassionate Registered Nurse (RN) to provide skilled nursing care to patients in their homes and communities. The RN will assess patient needs, develop and implement individualized plans, administer medications, and coordinate care with physicians and other professionals.

This field-based, per diem position requires travel to patients’ homes. The role emphasizes discharge planning, case management when assigned, and supervising home health

Qualifikationen

• Graduate of an accredited school of nursing.
• Current, active RN license in California.
• 1–2 years of recent acute care experience in an institutional setting.
• Current CPR certification.
• Valid driver's license and access to a reliable vehicle.
• Excellent observation and clinical assessment skills.
• Strong verbal and written communication skills.
• Effective problem-solving skills and basic mathematical skills.
• Ability to perform nursing duties according to established competency requirements.
• Ability to work independently and coordinate patient care responsibilities.
• Ability to perform physical duties of patient care.

Aufgaben

• Conduct initial patient and family assessments to determine home health care needs.
• Regularly reassess patients’ nursing needs and evaluate condition changes.
• Initiate and revise care plans as patient needs change.
• Develop care plans with patient-centered goals and nursing interventions.
• Provide patient education based on assessments and plan of care.
• Coordinate with physicians and healthcare professionals for patient care.
