Qualifications

Responsibilities

• Coordinate care plans and monitor progress to achieve appropriate health outcomes.
• Evaluate member needs, eligibility and coordinate services with providers.
• Provide coaching and support to empower member engagement and healthy lifestyle choices.
• Collaborate with case managers, supervisors and Medical Directors on care plans.
