Job summary

CVS Health is seeking a Case Management Analyst to coordinate care for members across the Zion, Waukegan, Beach Park and surrounding areas in Illinois. Qualifications

• Minimum 2 years of experience in behavioral health, social services or human services field.
• Minimum 2 years of case management experience.
• Willing and able to travel up to 50-75% of the time to meet members face to face in Rockford, IL and surrounding areas.
• Reliable transportation; mileage reimbursed per policy.
• Preferred: Case management and discharge planning experience.
• Preferred: Managed Care experience.
• Preferred: Microsoft Office experience.

Responsibilities

• Facilitate healthcare outcomes through care coordination, support and education for members.
• Evaluate member needs and recommend a care resolution approach.
• Coordinate care plan activities and monitor progress.
• Escalate quality of care issues and secure appropriate options.
• Coach and empower members to engage in healthcare decisions and healthy lifestyle changes.
• Document care and ensure regulatory compliance.

Education

Bachelor's or non-licensed MS in behavioral health/human services
