Job summary

The Salvation Army Ferguson Community Empowerment Center in Ferguson is hiring for a Pathway to Health Case Manager. This role will recruit, enroll, and apply appropriate interventions for clients in Pathway of Hope and Pathway to Health programs.

It is a community-based position with frequent local travel. Responsibilities include conducting intake, goal setting, home visits, and coordinating with partner agencies.

Qualifications

• Bachelor's degree in human services or related field with 1 year related work experience.
• Outgoing personality with strong communication and cross-cultural skills.

Responsibilities

• Recruit, enroll, and apply interventions for Pathway of Hope and Pathway to Health participants.
• Conduct pre-screening, intake, goal setting, and home visits.
• Maintain client records in SIMS & HMIS within 48 hours and report statistics.
