Job summary

The Salvation Army Ferguson Community Empowerment Center in Ferguson is hiring a Pathway to Health Case Manager. This individual will recruit, enroll, and apply appropriate intervention for clients in the Pathway of Hope and Pathway to Health programs. Position is community based with frequent local travel.

This is a full-time role (40 hours/week) with a $46,000 annual salary. Location at 9420 W Florissant Ave, Saint Louis, Missouri.

Qualifications

• Bachelor’s degree in human services field (BSW/MSW preferred) with one (1) year related work experience.
• Outgoing personality with good communication skills and ability to work with diverse cultures.
• Strong organizational skills; knowledge of Salvation Army and resources is helpful.
• Must have working knowledge of computers.

Responsibilities

• Recruitment activities to identify potential participants and build relationships with outside agencies for referrals.
• Conduct pre-screening and intake using assessment tools; set goals with clients; conduct follow-up meetings.
• Monitor goal attainment and perform home visits regularly with Pathway families.
• Distribute vouchers or checks as available and refer to other agencies if needed.
• Record client information in SIMS & HMIS databases within 48 hours and generate reports.
• Network with community agencies to maximize services and maintain a current resource guide.
