Job summary

Bluestone is seeking a Geriatric Complex Care Manager to manage high-need patients in senior living communities across Minnesota and surrounding areas. This field-based role connects patients, families, and primary care providers to execute unified care plans.

Primary responsibilities include reducing hospitalizations, coordinating with care teams, and ensuring high-quality, compliant care while working mostly in the field (90%).

Qualifications

• Bachelor's degree or higher preferred; licensed personnel preferred (LPN, RN, or SW).
• 3–5 years in value-based care, population health, care coordination or discharge planning.
• Experience in behavioral health preferred.

Responsibilities

• Care coordination across senior living communities to manage complex chronic conditions.
• Develop and manage individualized care plans with families and providers.
• Collaborate with MDs, NPs, and PAs to update care strategies.
• Lead transitions from hospital to home and reduce 30-day readmissions.
• Ensure regulatory-compliant care management and field-based efficiency.
