Job summary

Bluestone Physician Services is seeking a Care Manager who delivers individualized, 1:1 care for individuals with disabilities and persistent mental health conditions living in group homes and assisted living settings.

The role centers on building strong relationships with patients, families, and Primary Care Providers while executing unified care plans to improve health outcomes and reduce avoidable hospitalizations. 90% field-based work throughout MN, WI, and FL with daytime hours.

Qualifications

• Bachelor’s degree or higher preferred.
• Licensed personnel preferred - LPN, RN, or Social Worker.
• 3-5 years of experience in value-based care, population health, care coordination or discharge planning.

Responsibilities

• Care Plan Development and management for individuals with disabilities.
• Behavioral health management and CoCM model implementation.
• Coordinate transitions from hospital/rehab and support 30-day readmissions prevention.
• Collaborate with physicians and clinicians for real-time patient updates.
• Ensure care plan adherence and regulatory compliance.
