Job summary

Bluestone is hiring for a Care Manager to support individuals with disabilities and persistent mental health conditions in group homes and assisted living settings. You will drive 1:1 relationship building, health outcomes, and care gap closure while partnering with Primary Care Providers.

The role is 90% field-based with 8am–5pm Mon-Thu and 8am–3pm Fri, in Minnesota communities; benefits include comprehensive health coverage and a 60+ day overall schedule.

Qualifications

• Experience with value-based care and population health.
• Proven ability to develop and manage care plans.
• Strong interpersonal and written communication skills.

Responsibilities

• Care plan development and management for individuals with disabilities.
• Execute behavioral health interventions within the CoCM model.
• Collaborate with MDs, CNPs, and PAs for updated care strategies.
• Close clinical gaps, support VBC contracts, and manage utilization.
• Lead hospital and rehab discharge coordination and transitions.
• Coordinate telehealth services to ensure timely care.
• Educate families and facility staff on Bluestone care model.
