Bluestone is seeking a Geriatric Complex Care Manager to support complex, chronically ill patients in senior living communities across Minnesota. You will serve as the primary link between patients, families, and the Primary Care Provider, ensuring a unified care plan and reducing hospitalizations.

This field-based role is 90% in the community, with a schedule of 8am-5pm (Fri shorter), and focuses on care coordination, transitions, and education for families and facility staff, while adhering to

Qualifications

• Bachelor's degree or higher; licensed personnel preferred (LPN, RN, or Social Worker).
• 3–5 years in value-based care, population health, care coordination or discharge planning.
• Experience in behavioral health preferred.

Responsibilities

• Care Plan Development and management for individualized plans.
• Coordinate transitions and prevent 30-day readmissions.
• Discharge coordination and telehealth facilitation to timely interventions.
• Educate families and facility staff about dementia and Bluestone model.
• Ensure regulatory compliance and program integrity of care management.
