Job summary

Bluestone is hiring a Care Manager to provide individualized, 1:1 care for people with disabilities and persistent mental health conditions in group homes and assisted living settings.

You will serve as the primary contact among individuals, families and primary care providers, ensuring a unified care plan, closing care gaps, and preventing avoidable hospitalizations, with 90% field-based duties across Minnesota communities.

Qualifications

• Proven experience working with individuals with disabilities and persistent mental health conditions in residential care environments.
• Bachelor's degree or higher preferred; Licensure preferred (LPN/RN/Social Worker).
• Strong communication and organizational skills; ability to work independently.

Responsibilities

• Care Plan Development: Create individualized care plans aligned with standards and program requirements.
• Behavioral Health Management: Implement CoCM model and behavioral interventions.
• Clinical Partnership: Collaborate with MDs, NPs, PAs for real-time updates to care strategies.
• Gaps-in-Care & Utilization Management: Close clinical/documentation gaps and support VBC contracts.
