Job summary

Bluestone Physician Services is seeking a Care Manager specializing in disability and behavioral support to deliver 1:1, field-based care across Minnesota. You will build comprehensive care plans, coordinate with physicians, and close care gaps to reduce hospital utilization.

The role emphasizes direct patient care 90% of the time, with typical hours 8am–5pm Mon–Thu and 8am–3pm Fri, in Minnetonka and surrounding areas. Salary range disclosed and experience in behavioral health valued.

Qualifications

• Bachelor's degree or higher preferred; licensure as LPN, RN, or Social Worker preferred.
• 3–5 years of value-based care, population health, care coordination or discharge planning.
• Experience in behavioral health and residential care environments preferred.
• Strong time-management and ability to work independently.
• Proficiency with EHRs and computer tools.

Responsibilities

• Care Coordination: develop and manage individualized care plans.
• Behavioral health management using CoCM model.
• Collaborate with MDs/NPs/PAs for real-time updates to care strategies.
• Close clinical/documentation gaps to support value-based contracts.
