Job summary

Bluestone Physician Services seeks a Geriatric Clinical Care Manager (RN, LPN, or LSW) to support chronically ill patients in senior living communities across Minnesota. The role focuses on 1:1 patient management, care planning, behavioral health support, and collaboration with primary care providers to reduce hospitalizations.

This full-time position is 90% field-based with daytime hours, primarily in Apple Valley, Lakeville and surrounding areas.

Qualifications

• Bachelor's degree or higher preferred.
• 3–5 years of direct clinical experience in geriatric care, acute care, post-acute care, or health system discharge planning.
• Behavioral health experience preferred.

Responsibilities

• Provide care coordination and develop individualized care plans.
• Collaborate with MDs, NPs, and PAs for care updates.
• Support transitions from hospital to home and prevent hospitalizations.
• Engage patients and families to ensure effective, compliant care.
