Bluestone Physician Services seeks a Geriatric Clinical Care Manager (RN, LPN, LSW) for a 90% field-based role across Minnesota communities. The position emphasizes managing complex, chronically ill patients, coordinating with care teams, and ensuring consistent care plans with families and primary care providers.

The role features a 8am–5pm schedule (Mon–Thu) and 8am–3pm on Fridays, with a strong focus on reducing hospitalizations and optimizing patient outcomes.

Qualifications

• Bachelor's degree or higher preferred.
• Strong preference for 3–5 years of direct clinical experience in geriatric care or related fields.
• Experience in behavioral health preferred.

Responsibilities

• Care plan development and management to align with program requirements.
• Lead transitions to prevent 30‑day readmissions and coordinate hospital/rehab discharges.
• Educate families and facility staff about dementia, mental health, and Bluestone care model.
