Qualifications

• Bachelor’s degree in social work or behavioral science preferred.
• Two+ years of adult case management experience.
• Computer literate; proficient with Microsoft Office, Outlook, and internet.
• Valid Oklahoma driver’s license and proof of insurance.
• ODMHSAS Case Management Certification and SOAR Training preferred.

Responsibilities

• Provide field-based/mobile case management to persons experiencing long-term homelessness.
• Maintain accurate documentation in HMIS for every contact or collateral contact.
• Assist tenants with leases and obtaining IDs and other required documents.
• Assist tenants in accessing entitlements and benefits.
• Support HUD grant compliance via Individual Service Plans.
• Participate in Coordinated Case Management with partner agencies.
• Collaborate on activities with employment, financial education, and behavioral health services.
• Assist tenants in achieving housing stability and self-determination.
• Transport clients as needed and engage with community resources.
