Job summary

Optimal Case Management in Westminster, MD is seeking a Case Manager / Coordinator of Community Services (CCS) for a full-time, community-based role. You will partner with individuals with intellectual and developmental disabilities and families to understand needs, coordinate services, and advocate for progress toward meaningful life goals.

This position requires travel across the service region, in-home visits, and collaboration with providers, agencies, and community partners.

Qualifications

• Possess a Bachelor’s or graduate degree in social work or another health or human services field.
• Complete a supervised field experience in case management, behavioral health, or vocational rehabilitation as part of the degree or state’s recognition requirements.
• Two (2) years of full-time equivalent experience providing direct clinical care to the consumer.

Responsibilities

• Develop relationships with individuals and families and coordinate services around their needs, preferences, choices, and goals.
• Coordinate and monitor services across providers, agencies, community resources, and other support systems.
• Connect individuals and families with healthcare, housing, employment, housing, educational, financial, and social resources.
• Maintain accurate, timely, and person-centered documentation of case management activities.
• Manage an assigned caseload while meeting quality, timeliness, productivity, and compliance expectations.
