Job summary

Optimal Case Management seeks a dedicated Case Manager/Coordinator of Community Services (CCS) to support individuals with intellectual and developmental disabilities across community and home-based settings in the Baltimore area. This full-time role requires travel within the assigned region and strong documentation practices.

You will build person-centered relationships, coordinate services, and advocate for clients to achieve meaningful goals while maintaining compliance and quality standards.

Qualifications

• Possess a Bachelor's or graduate degree in social work or another health or human services field.
• Complete a supervised field experience in case management, behavioral health, or vocational rehabilitation as part of the degree or state's recognition requirements for permitting legally authorized practice, or two (2) years of full-time equivalent experience providing direct clinical care to the consumer.

Responsibilities

• Build trusting, person-centered relationships with individuals and their families or support systems.
• Learn what matters most to each person and help translate their goals into meaningful supports and services.
• Coordinate and monitor services across providers, agencies, community resources, and other support systems.
• Connect individuals and families with appropriate community, healthcare, employment, housing, educational, financial, and social resources.
• Monitor whether services are meeting the individual's identified needs and goals, and advocate for the individual when barriers to services or supports arise.
• Participate in planning meetings and collaborate with individuals, families, providers, State agencies, and community partners.
• Maintain accurate, timely, and person-centered documentation of case management activities in accordance with organizational and regulatory requirements.
• Manage an assigned caseload while meeting quality, timeliness, productivity, and compliance expectations.
• Participate in supervision, training, quality review, and professional development.
• Maintain confidentiality and protect sensitive information.
• Perform other duties reasonably related to the position.

Education
