Job summary

Optimal Case Management is seeking a Case Manager / Coordinator of Community Services (CCS) to work in Westminster, MD. This full-time professional role combines community-based fieldwork with home-based administrative tasks and regional travel.

Salary ranges from $58,240 to $60,320 annually, with in-person engagements and office documentation. The role emphasizes person-centered planning, advocacy, and diligent case management.

Qualifications

• Bachelor’s or graduate degree in social work or related field.
• Two (2) years of full-time equivalent experience providing direct clinical care or supervised field experience as part of degree or state requirements.

Responsibilities

• Build trusting, person-centered relationships with individuals and their families.
• Coordinate and monitor services across providers, agencies, and community resources.
• Connect individuals and families with appropriate community, healthcare, employment, housing, educational, financial, and social resources.
• Monitor services to ensure needs are met and advocate when barriers arise.
• Maintain accurate, timely, and person-centered documentation of case management activities.
• Manage an assigned caseload with quality, timeliness, productivity, and compliance.
