Responsibilities

• Conduct home visits with high-risk members to identify barriers to care, including home safety, transportation, employment needs, and social determinants of health (SDOH).
• Connect members with community resources, healthcare services, and social support programs.
• Conduct non-clinical assessments to identify needs and help resolve concerns.
• Perform telephonic and in-person outreach to engage hard-to-reach individuals and families.
• Build and maintain relationships with community partners and local resources.
• Coordinate and participate in community outreach events across Georgia.
• Support nurses, social workers, and clinical team members through outreach and member education.
