Job summary

Astyra Corporation seeks a Community Outreach Coordinator to connect individuals with healthcare providers, community organizations, and local resources in the assigned region. This field-based role focuses on guiding high-risk members through transitions in coverage and care.

You’ll build trust with community partners, educate members about resources, and proactively address barriers to care while maintaining strong documentation and confidentiality. Local travel is required.

Qualifications

• 1–3 years of community-based experience.
• Experience as a Patient Navigator, Community Health Worker, Case Manager, Social Worker, or similar role.
• Experience developing relationships with community organizations, healthcare providers, or other local stakeholders.
• Strong relationship-building and communication skills.
• Knowledge of community resources and ability to connect individuals with services.
• Cultural competency and ability to build trust with diverse communities.
• Strong organizational skills and attention to detail.
• Ability to take initiative, problem-solve, and work independently in the field.
• Ability to manage multiple priorities and changing needs.
• Comfortable collaborating with multidisciplinary healthcare and community partners.
• Willing and able to travel locally throughout the assigned region.

Responsibilities

• Build and maintain relationships with healthcare providers, community-based organizations, community health workers, and other local stakeholders.
• Develop outreach strategies that meet members where they are and create a trusted environment for healthcare engagement.
• Help members stay connected to healthcare and community resources during transitions in coverage and care.
• Identify potential gaps in care and help connect members with appropriate services and resources.
• Develop and strengthen a network of community partners throughout your assigned region.
• Collaborate with physicians, care coordinators, community health workers, case managers, pharmacists, social workers, educators, and other partners.
• Educate members about available community resources and advocate for their needs.
• Help members identify appropriate healthcare and support services and access them in a timely manner.
• Identify and proactively address barriers that may prevent members from accessing care or engaging with available services.
• Maintain communication with members, providers, community partners, and other appropriate parties.
• Maintain accurate and timely documentation, status updates, and required notifications.
• Protect confidential member information and follow organizational, state, and federal privacy requirements.
• Travel locally throughout your assigned region to meet with members, providers, and community partners.
