Job summary

Astyra Corporation is seeking a Community Outreach Coordinator to connect individuals with healthcare providers, community organizations, and local resources across their assigned region. This field-based role supports members transitioning from managed care to fee-for-service coverage, focusing on helping high-risk members stay connected to care and resources.

Qualifications

• 1–3 years of community-based experience.
• Experience as a Patient Navigator, Community Health Worker, Case Manager, Social Worker, or similar role.
• Ability to develop relationships with community organizations and local stakeholders.
• Strong organizational skills and attention to detail.

Responsibilities

• Build and maintain relationships with healthcare providers, community-based organizations, and other local stakeholders.
• Develop outreach strategies to meet members where they are and create a trusted environment.
• Help members stay connected to healthcare and community resources during transitions in coverage and care.
• Identify gaps in care and connect members with appropriate services.
• Develop and strengthen a network of community partners across the region.
• Collaborate with physicians, care coordinators, CHWs, case managers, pharmacists, social workers, educators, and other partners.
• Educate members about available resources and advocate for their needs.
• Help members identify and access appropriate healthcare and support services.
• Address barriers to care and maintain ongoing communications with members and providers.
• Maintain documentation and required notifications in a timely manner.
