Qualifications

• 1–3 years of community-based experience.
• Experience as a Patient Navigator, Community Health Worker, Case Manager, Social Worker, or similar role.

Responsibilities

• Build and maintain relationships with healthcare providers, community-based organizations, and other local stakeholders.
• Develop outreach strategies to meet members where they are and foster engagement.
• Help members stay connected to healthcare and community resources during transitions in coverage and care.
• Identify gaps in care and connect members with appropriate services and resources.
• Develop and strengthen a network of community partners within the assigned region.
