Job summary

Astyra Corporation is seeking a Community Outreach Coordinator to connect individuals with healthcare providers, community organizations, and local resources in the San Diego region. This field-based role focuses on helping high-risk members transition from managed care to fee-for-service coverage and stay engaged with care and services.

Qualifications

• 1–3 years of community-based experience.
• Experience as a Patient Navigator, CHW, Case Manager, Social Worker, or similar.
• Experience developing relationships with community organizations, healthcare providers, or local stakeholders.
• Strong relationship-building and communication skills.
• Knowledge of community resources and ability to connect individuals with services.
• Cultural competency to build trust with diverse communities.
• Strong organizational skills and attention to detail.
• Ability to take initiative, problem-solve, and work independently in the field.
• Willing to travel locally throughout the region.

Responsibilities

• Build and maintain relationships with providers, organizations, and stakeholders.
• Develop outreach strategies meeting members where they are.
• Help members stay connected to healthcare and community resources during transitions.
• Identify gaps in care and connect members to services.
• Develop and strengthen a network of community partners.
• Collaborate with physicians, care coordinators, CHWs, case managers, pharmacists, social workers, educators.
• Educate members about available resources and advocate for their needs.
• Help members access healthcare and support services promptly.
• Identify and address barriers to care.
• Maintain communication with members, providers, partners.
• Keep accurate documentation and status updates.
• Protect confidential information per privacy requirements.
• Travel locally to meet members, providers, partners.
