Qualifications

• High School Diploma or GED required.
• Medical Assistant experience required.
• Call center experience required.
• Clearances (Act 33, Act 34, Act 73) required prior to starting.

Responsibilities

• Conduct face-to-face outreach to educate members on preventive health services and barriers to care.
• Coordinate with providers and community organizations to improve access to preventive health services.
• Assist with Health Risk Assessments, scheduling, and referrals.
• Identify barriers such as transportation, finances, childcare, and health literacy, and provide resources.
