Job summary

Cityblock Health is seeking a Community Health Partner to support a care coordination approach for members with complex social, behavioral, and medical needs. You will collaborate with RN Care Managers, manage a caseload, and help address barriers to health in the community.

The role requires 2+ years in care coordination or related field, strong communication, problem solving, and documentation. You will conduct assessments (PHQ-9, GAD-7, AUDIT, DAST-10) and connect members with community

Qualifications

• 2+ years of progressive experience in care coordination, community health, health promotion, or related field.
• Strong problem solving and judgment with ability to escalate clinical concerns when needed.
• High quality written, verbal and presentation communication; experience with documentation.

Responsibilities

• Collaborate with RN Care Managers to support members with complex medical and behavioral health needs.
• Manage a caseload, addressing social, behavioral, and environmental barriers to health.
• Develop and implement care plans with members to achieve care goals.
• Conduct assessments (PHQ-9, GAD-7, AUDIT, DAST-10) and refer to behavioral health programs as needed.
• Assist with provider appointments, lab tests, and referrals to community resources to close quality gaps.
• Meet members in community settings (home, shelter, hospital) as needed; document interactions and tasks in care platforms.
