Job summary

CareSource is seeking a Community Health Worker to join the interdisciplinary care team, coordinating care for members and supporting their health goals. The role requires traveling to various settings, accompanying members to appointments, and assisting with care plan development and education.

You will verify eligibility, collect information, and help identify barriers to care while maintaining compliant documentation.

Qualifications

• High School Diploma or GED required.
• Minimum of two years of experience in community settings with at risk populations providing coordination of services preferred.

Responsibilities

• Engage with members in various settings to build a professional relationship.
• Accompany members to appointments and social service encounters when needed.
• Coordinate logistics to support care plan goals including reminders, transportation, and childcare arrangements.
• Verify eligibility, enrollment history, demographics and current health status.
• Contribute to assessments by gathering information from members, families, providers and other stakeholders.
• Assist in developing and implementing individualized care plans and report to the Case Manager.
• Identify and manage barriers to achieving care plan goals.
• Provide health education and wellness materials as directed by Case Managers.
• Maintain documentation per Care Management protocols and guidelines.
• Travel to member, provider and community-based visits as needed.
